PSG and French phenom Kylian Mbappe suffered a right ankle injury in the 26th minute of the French Cup Final on 7/24, at the hands of a very clumsy & dangerous challenge by Saint Etienne captain Loic Perrin. The injury PSG’s medical staff announced that Mbappe suffered a “serious right ankle sprain to the external lateral compartment” - which means ligaments on the outside aspect of his ankle suffered some degree of tearing - with re-evaluation set in 72 hours. With PSG’s sights now set on their 8/12 Champions League quarter-finals match vs Atalanta, the main question becomes: Can Mbappe make it back in time? Severity and return to play (RTP) Kylian’s return depends on multiple factors. No fracture Firstly, we can likely rule out a fracture because it wasn’t mentioned in the medical report and Mbappe was able to walk off the pitch after the injury and returned with a soft brace. Typically a hard CAM boot is used with a suspected ankle fracture. Further, a “serious” ankle sprain likely means Mbappe has either a grade 2 or grade 3 lateral ligament tear - typically the anterior talofibular ligament (ATFL). Grade 2 tear A grade 2 tear averages a return to play (RTP) of 2-3 weeks for elite level footballers. This would give Kylian a chance at playing vs Atalanta on 8/12, provided there’s no other confounding factors such as a bone bruise which can manifest later in physio and delay recovery. Grade 3 tear A grade 3 tear averages an RTP of 4-6 weeks for elite footballers. In this case, the best case would be Mbappe returning for a Champions League Final on 8/23 and worst case is continued signs of instability after six weeks which indicates the need for surgery, with an average return of 3-4 months. Long-term consequences The biggest concern for Mbappe after a severe ankle sprain is reinjury, with research indicating re-sprain rates as high as 80% and 72% developing signs of chronic instability.
